Form W-2 (Wage and Tax Statement)

If you did not receive your Form W-2, need a replacement copy, or require an additional copy, you may request one using the contact information below.

Once your request is received, you will be provided a W-2 Duplicate Request Form to complete.

The completed form must be returned with a copy of a valid government-issued photo ID. Requests cannot be processed without both items.

After the completed form and ID copy are received, a replacement Form W-2 will be issued using one of the following methods:

• Mailed to the address on the duplicate request form, or
• Picked up in person at the Admin office (photo ID required at pickup)

• Form W-2s cannot be emailed
• Processing time begins once all required documentation is received
• Incomplete submissions may delay processing
